The Royal College of Nursing (2018) explained that good communication can be hindered, by obstacles such as misunderstanding from patient or care staff. I encountered challenges with Mrs Paulson due to her limited English proficiency and Speech impairment; she finds it difficult to express the level of pain she was passing through. I went through Mrs Paulson’s observation sheet, and noticed that the hospital repositioning policy indicates that, patient should be repositioned every 2 hours whereas; she has not been repositioned for the past eight hours. I summoned courage and went to inform my Mentor; to avoid further deterioration of Pressure Ulcer. She came immediately and asked me to assist her to reposition Mrs Paulson. I offered her pressure relieve cushions to reduce pressure on her back.

The patient was annoyed because she does not like repositioning due to pain. However, NMC (2018) emphasised, that nurses should recognise and respond through the principle of therapeutic process to promote the patient well-being, personal safety and conflict resolution, I involved third person as an advocate as this might be necessary to encourage effective patient care and Adult Safeguarding (The statute Safeguarding Vulnerable Adult Act, 2006).

Diamond, (2015) Non-maleficence principle was applied to stop deterioration of the Pressure Ulcer and quick recovery. She might feel pain however, repositioning will prevent the deterioration (NICE, 2018).

 Conclusion: Professionalism in nursing care promotes patient centred care and encourages working in collaboration with other MDT, to enhance patient well- being.
